Your First Visit

Welcome to our office! Once your appointment has been scheduled you can expedite your first visit by completing your patient registration online. Simply click this link Patient Login to go to the “Patient Login” area. If you haven’t already received your login information please contact the office to get your user name and password.

Office hours are Monday- Thursday between 7am and 5pm.

After logging in you will need to complete the following sections: Patient Registration, Medical History and Consent Forms. History. Please remember to click the “Submit” button at the bottom of each section to save your information. We ensure that this web site is completely secure and your information will remain confidential. Here you may also find directions to our office and information about endodontic treatment.

Patient Billing

For your convenience we accept Visa, MasterCard, Discover, American Express, Care Credit and Cherry. We deliver the finest care at the most reasonable cost to our patients; therefore, payment is due at the time service.
If you have dental insurance, as a courtesy, we will help you with your insurance claims and will submit your claims electronically.  However, based on your specific coverage and insurance company, we will require you to pay a pre-determined portion for your services rendered.

Please bring your insurance information with you to the consultation so that we can expedite reimbursement. Most insurance companies will respond within four to six weeks. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
